To Our Patients:

New Patient:

  • We recommend that you download, print, and complete the “New Patient Registration Form” prior to your visit, to avoid prolonged wait time filling out paperwork in our office.

  • Please plan to arrive at least 30 minutes prior to your appointment to help us process paperwork quickly and get you in to see the provider right away. 

Senior Patient:

  • If you are a senior patient with chronic conditions, please bring a list of your current medications or the medication bottles for the provider to review.

     

Patient Portal Access:

  • If you would like to have access to your Electronic Medical Records, please ask our receptionist to activate your secure “Patient Portal” online by providing us with your email. 

What To Bring To Your Appointment: 

  1. Your driver’s license

  2. Your current insurance card

  3. All completed forms
