To Our Patients:
We look forward to seeing you during your next visit! Browse our list of Patient Forms and Advanced Directives to get started on you healthcare.
New Patient:
We recommend that you download, print, and complete the “New Patient Registration Form” prior to your visit, to avoid prolonged wait time filling out paperwork in our office.
Please plan to arrive at least 30 minutes prior to your appointment to help us process paperwork quickly and get you in to see the provider right away.
Our office includes a voluntary arbitration agreement in all new patient intake forms to support fair and efficient resolution of any potential disputes. Signing this agreement is part of our standard policy for establishing care. However, in compliance with state and federal regulations, we do not require this agreement from Medi-Cal or Medicare patients. For all other patients, if the agreement is declined, we may be unable to establish a doctor-patient relationship, and the patient may contact their health plan or IPA to choose an alternate provider who does not require arbitration.
Senior Patient:
If you are a senior patient with chronic conditions, please bring a list of your current medications or the medication bottles for the provider to review.
Patient Portal Access:
If you would like to have access to your Electronic Medical Records, please ask our receptionist to activate your secure “Patient Portal” online by providing us with your email.
