OUR HISTORY Founded in 2010, our organization has been committed to delivering comprehensive healthcare to patients with diverse needs, including chronic conditions, urgent issues, and routine care.
Since 2021, we have expanded access, care coordination, and equity-focused services, improving outcomes through an integrated, team-based care model.
We actively collaborate with partners to advance healthcare access, bridge gaps in care, and promote community well-being.
OUR TEAM CARE Our Team-Based-Care along with our providers and care coordinators, effectively addresses the needs of a large, diverse patient population.

LA COUNTY SPA2 Glendale and the surrounding areas are located within Service Planning Area (SPA2) of the Los Angeles County region, which consists of 8 areas in total.
PATIENT PORTAL Our patients can view their records online on PatientAlly, after activating their account with our staff.
