OUR HISTORY

  • Founded in 2010, our organization has been committed to delivering comprehensive healthcare to patients with diverse needs, including chronic conditions, urgent issues, and routine care.

  • Since 2021, we have expanded access, care coordination, and equity-focused services, improving outcomes through an integrated, team-based care model.

  • We actively collaborate with associates to advance healthcare access, bridge gaps in care, and promote community well-being.

OUR TEAM CARE

  • Our Team-Based-Care along with our providers and care coordinators, effectively addresses the needs of a large, diverse patient population.
