OUR HISTORY Founded in 2010, our organization has been committed to delivering comprehensive healthcare to patients with diverse needs, including chronic conditions, urgent health concerns, routine and preventive care.
Since 2021, Vida Family Practice PC has expanded access, strengthened team-based care, improved coordination, and advanced health equity through the BHEP community initiative led by Dr. Vigen V. Abovian.
Our team-based care improves patient outcomes, reduces barriers to care, and supports equitable access to needed services regardless of background, language, income, or other circumstances.
We collaborate with community partners and healthcare professionals to improve access to care, bridge gaps in services, and promote community well-being.
OUR TEAM CARE Our Team-Based-Care model brings together providers and care coordinators to effectively address the needs of our diverse patient population.

LA COUNTY SPA2 Glendale and the surrounding areas are located within Service Planning Area (SPA2) of the Los Angeles County region, which consists of 8 areas in total.
PATIENT PORTAL Our patients can view their records online on PatientAlly, after activating their account with our staff.
